Work leave

Documents required for formalization of the removal process or extension of removal for teaching staff

Long-term (full or partial) dismissal (Specialization, Master's, Doctorate, Post-Doctorate, more than 01 year).
1. Application for removal request to the Immediate Chief;
2. Completed Completion Form;
3. Letter from the host Institution of the course, confirming the candidate's acceptance, and translation, when it is the case of removal abroad;
4. Study Plan or activities to be carried out by the applicant;
5. Holiday Scale;
6. Discharge with the Library corresponding to the Campus to which it belongs;
7. Demonstration, by the Department or the Lotation Sector, of how the server will replace its activities, indicating the name of the substitute teacher or technician;
8. Certificate of the Permanent Commission for Accumulation of Positions and Jobs - CPACE, informing about the possible accumulation of positions and / or jobs;
9. Certificate of Time of Service, provided by PROGEP;
10. Letter of granting of funds from the funding body, with information regarding ticket, daily and other expenses;
11. Certificate of approval of removal by the Collegiate Department (for Teacher) or by the Immediate Chief of the stocking sector (for Technical-Administrative);
12. Certificate of Approval of removal by the Board of the Center (for Teachers) or by the Superintendent of the Supplementary Body (for Technical-Administrative);
13. Copy of the Removal Ordinance in case of Extension (TEACHER and ADMINISTRATIVE TECHNICIAN);
14. Certificate of approval from the Center Council (for administrative technician packed in the Centers);
15. Evaluation of the most recent annual performance, except for cases of very short-term leave (for technical-administrative), document issued by DGD / CDP / PROGEP attesting a satisfactory result in the most recent performance evaluation according to art. 10 of Law 11,091 / 2005;
16. Document proving the recommendation of the course by CAPES, when the course is held in Brazil or by the Ministry of Education or equivalent body, when the course is held abroad;
17. Certificate attesting that the server has not left by license to deal with particular matters or to enjoy license of qualification in the two years at the time of request of the removal.

 Obs: Note: From the request nº 14 only TECHNICAL-ADMINISTRATIVE documents are required.

  • Short-term leave (total or partial) (equal to or greater than 30 days and less than 01 semester).

1. Application for removal request to the Immediate Chief;
2. Completed Completion Form;
3. Letter from the host Institution of the course, proving the acceptance of the candidate, or letter of acceptance of work or invitation, and translation, when it is the case of removal abroad;
4. Study Plan or activities to be carried out by the applicant;
5. Demonstration, by the Department or the Warehouse Sector, of how the server will replace its activities;
6. Letter of granting of funds from the funding body, with information regarding ticket, daily and other expenses;
7. Certificate of approval of removal by the Collegiate Department (for Teacher) or by the Immediate Chief of the stocking sector (for Technical-Administrative);
8. Certificate of Approval of removal by the Council of the Center (for Teachers) or by the Director of the Supplementary Body (for Technical-Administrative).

  • Remaining of very short duration (30 days or less, granted in cases in which the lecturer or technician is accepted or invited to present scientific, cultural or technical work, teach a course, conference or attend a round table, event or Brazil and Abroad).

1. Application for removal request to the Immediate Chief;
2. Completed Completion Form;
3. Letter from the host Institution of the course, proving the acceptance of the candidate, or letter of acceptance of work or invitation, and translation, when applicable;
4. Demonstration, by the Department or the Lotation Sector, of how the server will be substituted in its activities, when it is a period exceeding 15 days;
5. Letter of granting of funds from the funding body, with information regarding ticket, daily and other expenses;
6. Certificate of approval of removal by the Collegiate Department (for Faculty) or by the Immediate Head of the stocking sector (for Technical-Administrative).

  • Training License

1. Completed Completion Form;
2. Application for a License for Training to the Immediate Chief, justifying the request;
3. Communication, by the Coordination of Legislation and Norms - Division of Rights and 4. Duties, of PROGEP, of the period of the quinquennium and date of acquisition of the right (TEACHER), Letter from the institution providing the course, proving the acceptance of the candidate, and translation, when it is the case of removal abroad (TEACHER / TECHNICIAN);
5. Approval by the Head of Department or Sector of the Study Plan or activities to be carried out, compatible with the function performed;
6. The process should be forwarded to PROGEP.

 

  • Ph.D. sandwich
    Extension of Past (From six months to Masters and one year to PhD).

    Application for a request for extension of removal from the Immediate Chief;
    Letter from the institution providing the course, justifying the extension;
    Specify the activities developed so far and the Study Plan or activities to be carried out by the applicant;
    Copy of Removal Ordinance;
    Letter of granting of funds from the funding body, with information regarding ticket, daily and other expenses;
    Certificate of approval of removal by the Collegiate Department (for Teacher) or by the Chief Immediate of the stocking sector (for Technical-Administrative);
    Certificate of Approval of removal by the Center Council (for Teachers) or by the Director of the Supplementary Body (for Technical-Administrative).
    Note: The removal process must be sent to the Pro-Rectory within a minimum of 90 days.
